The Alliance (including UW-Madison SHIP)
Blue Cross Blue Shield (including BCBS plans from all 50 states. You might see names from other states like: Anthem, Horizon, Wellmark, Regence)
Original Medicare
If yes, easy peasy - let’s get you scheduled here.
—If not, move to Step 2.—
Step 2
Not in-network, but still have a PPO or POS plan?
Check your insurance card for a PPO or POS plan. Most PPO and POS plans include out-of-network physical therapy benefits.
We’re happy to bill your plan out of network.
Patients are responsible for deductibles, copays, coinsurance, denied claims, and non-covered services.

Step 3
Does your insurance card say HMO or EPO?
We are not contracted with HMO or EPO plans.
If we send our claims to your insurance, they will deny these claims as a non-covered service.
But, that isn’t the end of the story.
We offer self-pay rates for our HMO people. And as an added bonus, if you have a FSA or HSA, physical therapy is considered an eligible medical expense for these types of accounts.
