We know insurance can be confusing, so we keep it simple.
We’re in-network with four insurance plans and submit claims for you.
If we’re out-of-network for your plan, you’ll be self-pay, but you may still be able to seek reimbursement from your insurance.
Our in-network insurance contracts:
Aetna
The Alliance (including UW-Madison SHIP)
Blue Cross Blue Shield (this includes BCBS plans from all 50 states. You may see names on your insurance card issued from other states like: Anthem, Horizon, Wellmark, Regence). We are out of network with Anthem Medicaid.
Original Medicare
Your out of pocket costs will depend on your benefit coverage (deductible, co-pays, co-insurance).
